Missing the "Add Library Package Reference" option

I have NuGet 1.7.30402.9028 installed in VS 2010 on a Windows 7 Professional PC.  Upon selecting Tools > Library Package Manager, the "Add Library Package Reference" option fails to appear.  I can see "Package Manager Console" and "Package Manager Settings" menu options, but that middle option does not appear.

Originally I had NuGet version 1.6.21205.9031 installed, and the "Add Library Package Reference" option did not appear with that installation either.

I downloaded the latest version of NuGet to install it, hoping that this missing menu item would appear after installing the latest.  After uninstalling that old version of NuGet (to bypass a "signature ... does not match" error), I was able to successfully install version 1.7.30402.9028.

But, the "Add Library Package Reference" option still does not appear.  Is there some additional extension that I need to install, or is there some option that I need to check or uncheck in Visual Studio?
